As an agent for the business listed above, I certify that the information contained in this Vendor Application is true. I understand that I am required to collect and report California State Sales Tax. I have read and understood the VENDOR CONDITIONS & POLICY STATEMENT enclosed and agree to abide by these conditions under penalty of removal from the festival without refund. I understand that there will be no refunds whatsoever. I understand that the cashing of my check does not constitute acceptance in the event, however, if I am not selected as a vendor I will receive a full refund. I expressly agree to indemnify and save and hold harmless the Doheny Blues Festival, its officers, agents, volunteers and employees from any and all claims for loss, damage, injury or liability of whatsoever nature and howsoever the same may be caused.
